Fax is the approved channel for referral records. Do not email patient information.

What your office can expect

Call our office to confirm current scheduling availability, communication practices, and reporting workflow for your patient's case. We can also explain how to reach the appropriate clinician with case-specific questions.

Coverage notes for your staff

✓ We accept Kaiser, Medicare, and PPO plans.
✓ We encourage all patients to have a doctor's referral before starting physical therapy. Requirements vary by insurance plan, so call us with your insurance card to confirm any authorization or documentation steps before scheduling.
✓We don't accept workers' compensation claims. We can still treat a work-related injury billed to the patient's own insurance or paid directly.
✓ Call us before referring motor-vehicle-accident or third-party lien cases so we can confirm whether the case can be accepted.
